Course Overview

This course presents an overview of POCUS evaluation of the first trimester of a pregnancy. Topics in this chapter include the clinical importance of confirmation of early gestation viability and age, the number of gestations, and other common variations and abnormalities, all of which can be determined by ultrasound. First-trimester ultrasound provides the best gestational age, which directly impacts clinical management throughout the pregnancy. Potential complications that can often be recognized during this time will be discussed, including failed or ectopic pregnancy and multiple gestation.

Course Objectives

  1. List indications for and essential components of performing POCUS in first trimester

  2. Describe how to ‘date’ an early pregnancy

  3. Describe the order of embryonic structures as they appear on ultrasound.

  4. List both suspicious and certain findings for early pregnancy loss (EPL)

  5. Recognize a subchorionic hemorrhage

  6. Recognize and differentiate anembryonic pregnancy and embryonic demise.

  7. Recognize multiple gestations and learn how to determine chorionicity and amnionicity

  8. Describe classic signs of a pregnancy of unknown location (PUL) and recognize sonographic clues to determine a PUL.

  9. Differentiate between a “ring of fire” on a corpus luteum cyst versus an ectopic pregnancy

  10. Describe types of ectopics, including cornual and c/section scar pregnancy
